My deepest apology for the actions of the staff member who was serving you, there is no excuse for what the staff member done. We have given the staff member a warning and they are reciving customer serve traning to prevent this from happening again. We want to give you a $50 gift card for anyone of our departments.

Hey, I think for the format you got to have more paragraphs but I like how you kept it short and got straight to the point. I feel like you havent made it sound like you are extremly sorry for Mr Jack Sampson like grovle to him more. Where you have said I want to give you $50 I think it might sound better if you say I would like to offer you a $50 gift card since you have had this horrible experience.
